what is a neoplasm ?

a neoplasm is defined as a new growth usually referring to a tumour

(An abnormal mass of tissue that forms when cells grow and divide more than they should or do not die when they should Neoplasms may be benign (not cancer) or malignant (cancer). Benign neoplasms may grow large but do not spread into, or invade, nearby tissues or other parts of the body.)

tumour cells vary in phenotype depending on what type of tumour cell they are, and by how far they are on in development

what does not always clinically benign mean?!?

it means that although it is benign at one point it may not always be benign it has the possibility to metastasise and become cancerous some benign tumours of the breast have a 50% possibility of becoming cancerous
